Clinical Characteristics of the Patients with Chronic Obstructive Pulmonary Disease in Chungbuk National University Hospital

Abstract
Purpose : The mortality of chronic obstructive pulmonary disease (COPD) is still increasing and its socioeconomic burden is enormous. This study was performed to evaluated the clinical characteristics of COPD patients who visited Chungbuk National University Hospital.

Materials and Methods : The COPD patients who visited Chungbuk National University Hospital were enrolled. Their clinical information were reviewed retrospectively.

Results : Forty eight COPD patients (46 men, 2 women) were enrolled. their mean age was 70 ¡¾ 8 year. Chief complaints were dyspnea (62.5%), sputum (18.8%) and cough (10.4%). Mean Medical Research Council (mMRC) dyspnea scale was 2.4 ¡¾ 1.4. Their spirometry showed FVC 90.8 ¡¾ 18.0%, FEV1 58.3 ¡¾ 23.0% and FEV1/FVC ratio 43.8 ¡¾ 12.3%. DLco was 103.9 ¡¾ 30.6%, and DL/VA was 83.6 ¡¾ 25.1%. mMRC score was correlated with FEV1/FVC ratio (r=-0.460, p=0.001) and DL/VA% (r=-0.870, p=0.001). Three patients did not receive either oral medication or inhaled medication. For the oral medications, xanthine derivatives were prescribed to 77%, beta2-agonist to 67% and mucolytics to 63% of patients, respectively. Fifteen percent of patients have taken the intermittent oral corticosteroid. For the inhaled medications, short-acting beta2-agonists were prescribed to 48%, short-acting anticholinergic to 15%, long-acting beta2-agnoists to 38%, long-acting anticholinergic to 33%, and inhaled corticosteroid to 38% of patients, respectively. Long-term oxygen therapy was prescribed to 3 patients. Older age, low FEV1/FVC ratio and mMRC dyspnea scale were associated with hospital admission.

Conclusion : Most of COPD patients who visited Chungbuk National University Hospital were men and their mean age was 70 years. Most common chief complaint was dyspnea. Stage 2 COPD patients with mean FEV1 58% was the commonest. Most of patients received xanthine derivatives for oral medication and short-acting bronchodilator for inhaled medication. Older age, low FEV1/FVC ratio and high dyspnea scale were associated with hospital admission for respiratory problems.
